Cardiovascular Health Assesment Program (CHAP) Laval

 

For many years, studies were held for the Cardiovascular Health Assesment Program (CHAP). CHAP is a community program gathering together family physicians, nurses, volunteers, organisms and adults of the community. He was launched to help with the prevention and management of chronic illnesses. Many studies have shown that CHAP is an effective intervention to reduce hospitalisation rates for cardiovascular diseases, to lower blood pressure among participants, to favor the adoption of an healthy lifestyle and to optimize medication regimens. To face the augmentation of chronic illnesses in Laval and to better coordinate the care provided to patients, the Centre de santé et de services sociaux (CSSS) de Laval recently elaborated  a unique clinic programmation, called Cible Santé, networked in all the city. In anticipation of the deployment of Cible Santé, expected at winter 2015, we want to hold a pilot project in two Family Medicine Group (FMG) of Laval; Médi-Centre Chomedey and Cité-de-la-santé. We invite you to look at the ressources and community services in Laval area that are linked to the adoption of an healthy lifestyle and to some chronic illnesses.
