Following the guidelines from Hypertension Canada, CHAP uses the following standards to determine actions required at a CHAP session.

Notify the onsite nurse supervisor. After the nurse supervisor has met with the participant, inform the participant about healthy lifestyle habits that can help lower blood pressure |
Inform the participant that their pressure is higher than normal, but that it does not represent an imminent danger to their health. Recommend: that they tell their family doctor. Recommend: that they go to the emergency department if, in the coming weeks, they develop symptoms such as visual disturbances, unconsciousness, severe vomiting, chest pain, severe shortness of breath, or other abnormal symptoms. If the systolic pressure is between 160 and 180, take the blood pressure a second time. If the blood pressure is still in the same range: Advise the participant to make an appointment with their family doctor. If the participant does not have a family doctor, encourage them to contact Health Care Connect. If necessary, send them to the nearest urgent care clinic. |

1. Measure the participant’s blood pressure again by first removing the blood pressure cuff and positioning it again. Ensure that it is placed correctly and that the participant is in the proper sitting posture– on a chair with feet flat on the floor, legs uncrossed and back supported2. Complete a nurse assessment form.3. Based on this nurse assessment results, immediately send to the nearest emergency room.
