CHAP STEPS TO SUCCESS:
Welcome to the Cardiovascular Health Awareness Program. (CHAP). This is a brief overview of the steps required to initiate and maintain a successful program in your community. Please note this is meant only as a general guide, and can be modified to suit your capacity and the unique needs of your community.
STEP ONE: COMMUNITY ASSESSMENT:
A) Identify Local Lead Organization
B) Hire a Local CHAP Coordinator
C) Create a Community Profile
D) Develop a Communications Plan
STEP TWO: LAYING THE GROUNDWORK:
E) Involve Local Physicians and Pharmacists
F) Identify Community Settings to host CHAP sessions
G) Recruit CHAP Volunteers
STEP THREE: SESSION PREPARATION:
H) Hire a Public Health Nurse((A ‘Designated CHAP Health Care Professional’ is anyone whose scope of practice includes assessing blood pressure. This may include a pharmacist, physician, nurse, nurse practitioner or registered practical nurse (RPN). In other CHAP communities, a public health nurse has filled this role.))
I) Develop a CHAP Session Schedule
J) Invite Participants to CHAP Sessions
STEP FOUR: DELIVERY AND MAINTENANCE:
K) Train CHAP Volunteers
L) Hold CHAP Sessions
M) Manage CHAP data and Evaluate
